The template contains basic categories for participants to fill out:
  • Name
  • Company/Organization Affiliation
  • E-mail Contact Info
  • Extra Column for whatever you might need for your event
The template is in Microsoft Excel format.  Edit and change the template as much you need to suit the needs of whatever conference, symposium, meeting, convention, forum, seminar or delegation you're hosting.

What to Write in a Coworkers Birthday Card


There's nothing is worse than receiving a birthday card filled with nothing but 10-15 carbon copy messages of "Happy Birthday, Person X!!" from your coworkers (or they merely sign their name...), but we all know how awkward it can be finding the right thing to say without sounding creepy or like you're trying way too hard.
Here are some quick and painless birthday card message options you can use to keep that card moving and off your desk:
  • Best wishes on your birthday for good health and happiness throughout the year.
  • Warmest wishes on your birthday, I hope you spend some time treating yourself!
  • Here's to another 365-day journey around the sun. Enjoy the trip!
  • I wish you all the best on your special day!
  • It's a pleasure working with you, I hope you have a special birthday.
  • Have a kick-ass birthday!
  • Hope your special day is filled with all the things that mean the most to you.
You might also want to mix it up a bit by writing "happy birthday" in a different language, ideally one that the birthday girl or birthday boy is familiar with -
  • Spanish: Feliz cumpleaƱos!
  • French: Joyeux anniversair!
  • Hebrew: Yom hu'ledet sameach!
  • German: Alles Gute zum Geburtstag!
  • Latin: Fortuna dies natalis!
  • Italian: Buon Compleanno!
  • Norwegian: Gratulerer med dagen!
  • Albanian: Urime ditelindjen!
  • Icelandic:Til hamingju med afmaelisdaginn!
If you really want to sound like an original smarty pants in the birthday card, go on to Wikipedia and find something cool to write about that happened on the day the person was born, simply searching by day.
For example, if your coworker is born on April 20th, don't go out of your way to mention that he or she she shares a birthday with Adolf Hitler, the worst person of all time ever.  Instead, perhaps mention that on that day in 1870, The YWCA was founded in New York City, which is much more pleasant. This works best when the event is actually somewhat relevant in the life of the birthday boy or girl.
Or say you have a coworker unlucky enough to be born on September 11th.  Instead of referencing the horrible terrorist attack that occurred on that day in 2001, maybe instead let your coworker know that he or she shares a birthday with national treasure Harry Connick, Jr. and rapper Ludacris.
